Patient Bill of Rights
Upon admission to CCMH you will be offered a copy of a “Patient Bill of Rights.” If you or your immediate family member have any questions regarding your rights as a patient, please contact Social Services.
Authorization to Release Information
Your registration materials include a form that authorized CCMH to release information to your insurance company. Accurate and complete information help assure that your claim will be promptly paid. Please complete a new Authorization and Release Information any time your coverage changes.
HIPPA/Patient Confidentiality
PROTECTING YOUR PRIVACY CCMH is committed to protecting the privacy of all patients. This includes all personal information and medical records. We make full compliance with all current HIPPA regulations. If you have questions or concerns regarding your privacy at CCMH please ask for the HIPPA officer.
Tips
Our employees are professionally committed to making your visit as comfortable as possible. We are pleased to be of service to you. No employee shall accept tips or other gratuities. Please do not offer gifts or gratuities to our staff members.
