Introducing the Community Initiatives Fund

The CCMH Foundation is proud to launch the Community Initiatives Fund, a new program dedicated to supporting projects that improve health and wellbeing in our region. Through this fund, local organizations can apply for mini grants to address critical needs such as social determinants of health, mental health and substance use, and chronic disease prevention.

Who Can Apply?

Eligible applicants include public entities, registered nonprofits, and community organizations located in or serving residents of Carroll, Chariton, Livingston, and Lafayette Counties. Projects must focus on one or more priority areas:

  • Social Determinants of Health (e.g., transportation, housing, food access)
  • Mental Health (including substance use disorder)
  • Chronic Disease Prevention (e.g., obesity, diabetes, heart disease)

Eligible entities are encouraged to apply. Decisions are made on a rolling 60-day application period. Awardees will be contacted using the information provided on the application.
