From Hospital to Home—CCMH Supports Your Recovery Journey

CARROLLTON, Mo. – Carroll County Memorial Hospital offers short-term, post-acute care for patients who no longer require acute hospital services but need skilled care or therapy before safely returning home. We partner with hospitals to provide continuity of care, reduce re-admissions, and support recovery.
When patients reach a point in their recovery that acute care is no longer necessary, but they are not quite ready to return home, they can be evaluated for this special program that provides skilled follow-up care. Swing bed care is a wonderful option that is particularly helpful whenever a patient needs only a few more days or weeks in order to reach their recovery goals. Even if the original qualifying hospital stay occurred elsewhere, patients still have the ability to receive swing bed services at Carroll County Memorial Hospital.
Ruth Owen spent a few weeks at CCMH recovering after her 10-hour surgery at Boone Hospital.
“Anybody can see I’ve made great progress,” Owen said. “Every aspect that I utilized was excellent!”
Services offered through the swing bed program include skilled nursing care, therapy services, wound care, IV therapy, and comprehensive care coordination.
“I had more therapy done here than post care,” Owen said. Previously, she was only receiving therapy once a day. When she came to CCMH, she saw therapy twice a day.
“The important thing to know is you can receive post-surgery care right here,” Owen said. “The nursing staff was superb and considerate of all my visitors as well. The food was excellent. I’m a picky eater, but they really aim to please.”
CCMH partnered with Gates Hospitalist LLC in July 2024 to help our patients have medical care in the hospital while working together with their primary care physicians, consulting specialist physicians and the nursing staff.
“Dr. (Lancer) Gates and his colleague were engaged and concerned,” Owen said. “He diagnosed and treated issues that were not even assessed at the previous facility, and was thoughtful to involve my family as well.”
Utilizing CCMH’s swing bed program allows patients to continue care closer to home and loved ones, reduce their length of stay while ensuring continued quality care, and have a supportive environment for rehabilitation with an experienced team dedicated to patient-centered recovery.
