All co-payments are due at the time of registration. If co-payment is not collected at the time of registration, services may be rescheduled.

Diagnostic and Therapeutic Services

(including Laboratory Services, Rehab Services, Cardio Pulmonary Rehab Services and Radiology Services. Also registered here is Wound Care Clinic.)

Enter through Carroll County Memorial Hospital main door, registration will be down the hall on the left.  Check In on the CrossChx tablet at the welcome desk.

Surgical Services

Enter the CCMH Medical Plaza on the Lower Level and Check-In will be through the door and to the left.

Outpatient Specialty Clinics

(including Allergy & Asthma, Cardiology, ENT, Gastroenterology, Interventional Pain, Oncology/Hematology, Orthopedic, Podiatry, Pulmonology, Rheumatology, Tele-Nephrology, Urology and Vascular.)

Enter the CCMH Medical Plaza and take the elevator to Level One. Check-In will be to the left when coming off the elevator.  Check In on the CrossChx tablet at the welcome desk.
